Cardiac/Pulmonary Rehabilitation

Cardiac/Pulmonary Rehabilitation provides individually prescribed exercise, risk factor modification, education, and behavioral counseling by specially trained staff. The main emphasis is focused on assisting the patient to make positive lifestyle changes in order to reduce future cardiac/pulmonary complications or events.

Cardiac Rehabilitation Services:

  • Phase II Outpatient Program

Begins 1-2 weeks after an acute event (MI, open-heart surgery, aortic valve replacement, Stent/PTCA, and stable angina) and continues for approximately 8-12 weeks and includes the following:

    • Cardiac monitored exercise sessions
    • Individually designed exercise program and activity guidelines (includes walking, biking, resistance training, and other heart strengthening activities)
    • Education (coronary artery disease signs and symptoms, medications, stress management, blood pressure management, cholesterol, and obesity)
    • Dietary counseling
    • Diabetes education
    • Smoking reduction/cessation
    • Covered by insurance
  • Phase III H.O.P.E. (Healthy Opportunities for Personal Enhancement)

Begins 8-12 weeks after an acute event or discharge from the outpatient program.

    • Designed for anyone at risk for heart and lung disease:

Congestive Heart Failure                              Sedentary Lifestyle
Hypertension                                                   Diabetes
Peripheral Vascular Disease                       Obesity

    • Provides continued support for individuals and assists in setting short term/long term goals for continued reduction of risk factors associated with heart disease.
    • Self pay program ($6.00 per session)

 

Pulmonary Rehabilitation Services:

  • Phase II Outpatient Program

Begins 1-2 weeks after hospital discharge from an acute event (chronic obstructive pulmonary disease COPD, emphysema, pulmonary fibrosis, pneumonia, and chronic bronchitis) or with physician referral. The Phase II program is designed to return the patient to the highest level of functioning allowed by their pulmonary illness and includes the following:

    • Individually tailored exercise program (includes walking, biking, and resistance training)
    • Activity guidelines
    • Breathing retraining techniques
    • Hydration status
    • Dietary counseling
    • Stress management
    • Smoking cessation
